Bexley Health Neighbourhood Care CIC (BHNC) and our 21 GP practices use the Health Diagnostics platform to support delivery of our NHS Health Checks programme - covering everything from invites to data recording, reporting, and onward referrals. The team at HD are always on hand to support with any queries or platform changes. They’re approachable and consistently strive to meet our needs as one of the most responsive teams we work with in regard to shared development and learning. The platform itself is easy to use and makes it simple to extract and report information, especially for public health reporting on the number of health checks completed and their outcomes.

East Ridings

They are always on hand to support, answer any questions, walk through any changes that are needed on the dashboard, as well as offer their advice and expertise, especially when considering new developments to our Health Checks Programme here in Kent. The team are very responsive. We are very pleased with the dashboard created for the Health Checks Programme, which provides us with detailed data on how the Service is improving. The team are always looking at ways to improve the dashboard and our user experience.

Manchester

An advantage of using the software is its ability to streamline data collection and reporting, reducing errors and ensuring compliance with data protection regulations. The real-time access to data has been very helpful for monitoring and analysing trends in the uptake of the service, allowing for the identification of health inequalities and enabling us to use a targeted approach to reducing them. The software has improved the efficiency of data sharing with participants’ GPs, ensuring they receive the necessary follow-up care promptly. Overall health diagnostics has enhanced the effectiveness and accuracy of our workplace health checks.  

Medway

The Health Diagnostics team have been fantastic at modifying our Targeted Invites software to accommodate new project requirements, and the Technical Support Team provides brilliant training and support to all users across Medway. The Targeted Invites software has made inviting our eligible population for NHS Health Checks a slick and easy process. Previous systems were clunky and labour-intensive for practice staff, but the introduction of Targeted Invites has freed up capacity and empowered teams to manage the invitation process for their patient groups more effectively.

Merton

Working with Health Diagnostics has allowed Merton Health to create local referral pathways with one-click referrals to local providers, tailored to the person’s needs. This collaboration has saved significant time for the team and increased referrals to services like the NHS Diabetes Prevention Programme, smoking cessation, and alcohol support. Another key outcome has been the significant reduction in numbers of incomplete checks since our teams have been using the Health Diagnostics software for NHS health check documentation (from 59 in 2021-2022) to close to zero.
